SS-31 (Elamipretide, also designated Bendavia and MTP-131) is a cell-permeable, mitochondria-targeted tetrapeptide with the sequence D-Arg-2′,6′-dimethyltyrosine (Dmt)-Lys-Phe-NH₂. The peptide was designed with an alternating aromatic-cationic motif (aromatic-cationic-aromatic-cationic) that enables selective accumulation in the inner mitochondrial membrane (IMM), driven by the mitochondrial membrane potential (ΔΨm). This targeting achieves over 1,000-fold concentration in mitochondria relative to the extracellular space — making SS-31 one of the most efficiently targeted subcellular delivery molecules in peptide chemistry.

The molecular target of SS-31 is cardiolipin, a unique diphosphatidylglycerol phospholipid found exclusively in the inner mitochondrial membrane. Cardiolipin plays essential structural roles: it anchors and stabilizes electron transport chain (ETC) complexes I, III, IV, and V, stabilizes the supercomplex (respirasome) organization that enables efficient substrate channeling, and maintains the association of cytochrome c with the IMM surface necessary for electron transfer from Complex III to Complex IV. By binding to cardiolipin, SS-31 stabilizes these protein-lipid interactions, optimizing electron transfer efficiency, reducing electron leak to molecular oxygen (the source of mitochondrial superoxide), and preserving cristae ultrastructure.

Published research spans multiple disease-relevant paradigms: cardiac ischemia-reperfusion injury (cardioprotection), acute kidney injury (renal mitochondrial rescue), skeletal muscle aging (reversal of age-related mitochondrial dysfunction), heart failure (improved cardiac energetics), Barth syndrome research (cardiolipin remodeling deficiency), and neurodegenerative models.
