GHRP-6 (Growth Hormone Releasing Peptide-6) is a synthetic hexapeptide with the sequence His-D-Trp-Ala-Trp-D-Phe-Lys-NH₂, identified through systematic screening of Met-enkephalin derivatives by Bowers and Momany in the 1980s. It was one of the earliest synthetic growth hormone secretagogues (GHS) discovered, predating the identification of the endogenous ligand ghrelin by over a decade. This historical significance means GHRP-6 has accumulated an extensive body of published pharmacological data, making it a well-characterized reference compound for GH secretagogue research.

GHRP-6 functions as a potent agonist at the growth hormone secretagogue receptor type 1a (GHS-R1a), the same receptor subsequently identified as the target for endogenous ghrelin. GHS-R1a is a Gq/11-coupled GPCR that signals through phospholipase C, IP3 generation, and intracellular calcium mobilization in anterior pituitary somatotrophs. This calcium-dependent mechanism is distinct from and complementary to the cAMP/PKA pathway activated by GHRH, which is the molecular basis for the well-documented synergistic GH release observed when GHRP-6 and GHRH analogs are combined in research settings.

Unlike later-generation GH secretagogues such as Ipamorelin, GHRP-6 retains significant ghrelin-mimetic properties beyond GH release. Published research documents stimulation of appetite and food intake, acceleration of gastric motility, stimulation of cortisol and ACTH release, and mild prolactin elevation. These “off-target” effects from a GH-selective standpoint are actually valuable research features — they make GHRP-6 a useful tool for studying the broader physiological roles of ghrelin receptor activation, including gut-brain axis signaling, orexigenic pathways, and the relationship between appetite and GH secretion.
