Sermorelin Acetate (GRF 1-29 NH₂) is a synthetic peptide identical to the first 29 amino acids of human growth hormone-releasing hormone (GHRH 1-44), amidated at the C-terminus. Endogenous GHRH is a 44-amino acid peptide produced by arcuate nucleus neurons in the hypothalamus and released into the hypophyseal portal blood system to stimulate GH synthesis and secretion from anterior pituitary somatotrophs. Structure-activity studies established that the N-terminal 29 residues contain the complete receptor-binding and activation domain, with residues 30-44 serving primarily as a structural spacer that does not contribute to receptor affinity or efficacy.

Sermorelin maintains the native, unmodified human GHRH(1-29) sequence — it contains no D-amino acid substitutions, no acylation, and no bioconjugation. This makes it the reference standard for GHRH receptor (GHRHR) pharmacology research: it produces the most physiologically representative GHRH receptor activation available in a synthetic peptide. Sermorelin activates GHRHR (a class B GPCR on somatotrophs) through Gs-coupled adenylyl cyclase activation, cAMP elevation, and PKA-mediated GH gene transcription and granule exocytosis. The resulting GH release is pulsatile and subject to normal somatostatin feedback inhibition — distinguishing it from DAC-conjugated forms that produce tonic GH elevation.
