Semax (sequence: Met-Glu-His-Phe-Pro-Gly-Pro) is a synthetic heptapeptide derived from the ACTH(4-10) fragment of adrenocorticotropic hormone. The ACTH(4-10) sequence (Met-Glu-His-Phe-Arg-Trp-Gly) was identified in early neuroendocrine research as the “behaviorally active” core of ACTH — it modulates attention, learning, and memory in animal paradigms while completely lacking the adrenal-stimulating (steroidogenic) activity of full-length ACTH, which requires the N-terminal 1-24 sequence for adrenal receptor activation. Semax replaces the C-terminal Arg-Trp-Gly with Pro-Gly-Pro (PGP), a tripeptide extension that confers resistance to both aminopeptidase and carboxypeptidase degradation, extending the biological half-life from minutes to hours.

The neurotrophic mechanism of Semax has been extensively characterized in published research. The peptide upregulates BDNF (brain-derived neurotrophic factor) mRNA and protein expression in hippocampal and cortical neurons through CREB-dependent transcription. BDNF/TrkB signaling promotes neuronal survival, synaptic plasticity, long-term potentiation (LTP), and dendritogenesis. Semax also increases NGF (nerve growth factor) expression and modulates serotonergic neurotransmission by influencing SERT (serotonin transporter) expression and tryptophan hydroxylase activity. Dopaminergic effects have been documented through modulation of D1/D2 receptor signaling in striatal and prefrontal regions. Gene expression profiling studies have identified broad transcriptional changes affecting neuroplasticity-related genes, immune-related genes, and vascular genes in brain tissue.
